Why There Are Multiple GFR Formulas
Different GFR formulas were developed in different eras using different patient populations. The MDRD equation was developed in 1999 using data from CKD patients, which means it systematically underestimates GFR in healthy people (gives falsely low readings above 60). The CKD-EPI equation (2009, updated 2021) was developed using a broader population including healthy individuals, making it more accurate across the full range of kidney function.

Why the Race-Free Update Matters
The MDRD and original CKD-EPI equations included a coefficient for Black patients that multiplied eGFR by 1.21. This was based on the observation that Black individuals on average have higher muscle mass (and therefore higher creatinine), not a racial difference in kidney function. However, studies showed this systematically overestimated GFR in Black patients, leading to delayed CKD diagnosis, lower transplant referral rates, and worse outcomes. When Cockcroft-Gault Is Still Used
The Cockcroft-Gault equation (developed in 1976) is still used for medication dosing, especially for chemotherapy drugs and certain antibiotics. It estimates creatinine clearance rather than GFR, and it uses actual body weight rather than standardized body surface area. Always use CKD-EPI 2021 for CKD staging and Cockcroft-Gault for drug dosing.
